Oddly I have not been able to submit the survey. I get an error telling me to “fill out the survey questionnaire before submitting” but every question has an answer, including all the questions to “start” the survey (name, PC#, etc.)… ugh. Did this in IE and Firefox both – same issue. Anyone else have problems with it?

@Justin – It was a little fussy with me too but after a few tries I seem to have everything put in correctly. Give it another go! Maybe try another computer. I am using Google Chrome. If you want to email your PC#, name & birth date privately I’d be happy to try it on my end for you.

Thanks so much for your help. I really appreciate your offer to fill the survey in on my behalf, but won’t be necessary: I tried your suggestion of Chrome, and it worked! I did also notice I was filling my birthday in using U.S. formatting rather than European as the site asks (DD/MM/YYYY) … maybe that was the issue in the other browsers. (If so, then @IHG: just a heads-up, you could do a better job of high-lighting field errors with your form validation, otherwise users like me will mess it up!)
